{"id":2409,"date":"2026-06-19T19:10:59","date_gmt":"2026-06-19T19:10:59","guid":{"rendered":"https:\/\/silvybrand.com\/?p=2409"},"modified":"2026-06-19T19:10:59","modified_gmt":"2026-06-19T19:10:59","slug":"canada-missed-chances-titan-fatal-implosion","status":"publish","type":"post","link":"https:\/\/silvybrand.com\/?p=2409","title":{"rendered":"Canada Missed Chances to Inspect Titan Before Fatal Implosion"},"content":{"rendered":"<p><br \/>\n<\/p>\n<div>\n<p><span class=\"lead-in-text-callout\">A report from<\/span> Canada\u2019s Transportation Safety Board has highlighted regulatory failures that allowed OceanGate\u2019s unregistered, unflagged, and <a href=\"https:\/\/www.wired.com\/story\/titan-submersible-hearings-week-one-testimony-oceangate-implosion\/\" class=\"text link\">uncertified <em>Titan<\/em> submersible<\/a> to operate out St. John\u2019s, Newfoundland, for years before it <a href=\"https:\/\/www.wired.com\/story\/titan-sub-oceangate-hull-failure-loss-tragedy\/\" class=\"text link\">imploded on a tourist trip<\/a> to the wreck of the <em>Titanic<\/em> in 2023.<\/p>\n<p class=\"paywall\">\u201cWhen it came to the <em>Titan<\/em>, critical information existed across multiple federal government organizations, but no one was responsible for connecting the dots,\u201d says TBS chair Yoan Marier in a statement. \u201cWithout a complete picture of the operation, the <em>Titan<\/em> continued to operate in Canada without regulatory oversight.\u201d<\/p>\n<p class=\"paywall\">OceanGate first interacted with the Canadian government while <em>Titan<\/em> was still undergoing final assembly in Everett, Washington. In May 2021, Fisheries and Oceans Canada laid out plans to pay the company $25,000 to support deep-sea ecosystem research during missions to the <em>Titanic<\/em> the following year. But Global Affairs Canada denied OceanGate a research permit after the company claimed, inaccurately, that Fisheries and Oceans would act as its sponsor.<\/p>\n<p class=\"paywall\">The <em>Titan<\/em>\u2019s maiden voyage to the <em>Titanic<\/em> the next month was unsuccessful after one of its titanium domes fell off, and the ship carrying the sub, the <em>Horizon Arctic<\/em>, returned to St. John\u2019s. But before any of the disappointed passengers who had paid over $100,000 to see the wreck could disembark, the ship was directed to a secure lockdown area of the harbor. There, a team of armed officers from Canada\u2019s Border Security Agency boarded the <em>Horizon Arctic<\/em>. They interrogated the passengers about Covid-19 precautions and their role in the dives.<\/p>\n<p class=\"paywall\">\u201cThey were extremely intimidating,\u201d passenger Gary Philbrick tells WIRED. \u201cI couldn\u2019t get off the ship fast enough.\u201d<\/p>\n<p class=\"paywall\">The agents also asked why OceanGate was operating without a research permit. David Concannon, a lawyer who had worked with OceanGate in the past, told them that the <em>Titan<\/em> would only be diving in international waters, and the agents left. \u201cThey had zero interest in the sub. Absolutely none,\u201d he tells WIRED. \u201cThey were there to look at paperwork.\u201d<\/p>\n<p class=\"paywall\">That was correct, says Etienne Seguin-Bertrand, an investigator with the Transportation Safety Board: \u201cAs long as the sub had been imported properly and any applicable duties paid, it wasn\u2019t part of their mandate to make sure that it was properly registered and safe.\u201d<\/p>\n<p class=\"paywall\">Another agency, Transport Canada, is responsible for overseeing compliance with regulations for all vessels, including submersibles. These include requirements that vessels are registered, flagged, or certified, particularly if they are carrying passengers. It can inspect vessels and, if necessary, carry out enforcement. But Transport Canada had decided that the <em>Titan<\/em> was actually part of the <em>Horizon Arctic<\/em>\u2019s cargo and therefore not a vessel subject to inspection.<\/p>\n<p class=\"paywall\">In July 2021, a researcher from Fisheries and Oceans Canada traveled on a subsequent OceanGate mission as an observer. They reported back that the carbon fiber <em>Titan<\/em> had not been approved or certified by any regulatory body and was not carrying insurance. Their concerns never made it to Transport Canada\u2019s team that oversees marine safety, though <a data-offer-url=\"https:\/\/www.tsb.gc.ca\/eng\/medias-media\/communiques\/marine\/2026\/m23a0169-20260617.html\" class=\"external-link text link\" data-event-click=\"{&quot;element&quot;:&quot;ExternalLink&quot;,&quot;outgoingURL&quot;:&quot;https:\/\/www.tsb.gc.ca\/eng\/medias-media\/communiques\/marine\/2026\/m23a0169-20260617.html&quot;}\" href=\"https:\/\/www.tsb.gc.ca\/eng\/medias-media\/communiques\/marine\/2026\/m23a0169-20260617.html\" rel=\"nofollow noopener\" target=\"_blank\">the report<\/a> doesn\u2019t make clear where the disconnect was. Fisheries and Oceans never followed through with its plan to fund <em>Titan<\/em> missions.<\/p>\n<p class=\"paywall\">As OceanGate continued to operate from St. John\u2019s in 2021 and 2022, the <em>Titan<\/em> made successful dives to the <em>Titanic<\/em> and several sites within Canadian waters. The company eventually interacted with a total of 10 Canadian federal agencies, including Parks Canada, the Department of National Defense, and the Royal Canadian Mounted Police.
